Post Permanency Services
Continued Support After Adoption
Adoption doesn’t end at finalization and neither does support.
SWAN-funded Post Permanency Services are available at no cost to eligible Pennsylvania
Families and include:
- Assessment
- Case management and advocacy
- Respite services
- Support groups
- Education and referrals
These services help families understand how early trauma can affect behavior, relationships, and
emotional well-being and provide strategies to strengthen connection and promote healing.
